The Victorian Cancer Survivorship Community of Practice forum brings together health professionals with an interest in cancer survivorship and the implementation of survivorship models of care. The forum provides a valuable opportunity to explore trends in survivorship care, and to exchange insights and practical learnings.

This free event is open to anyone working with cancer survivors across all sectors, including hospitals, community and non-government organisations, research and policy.

Delivered as a joint initiative between the Australian Cancer Survivorship Centre (ACSC), Victorian Integrated Cancer Services (VICS) and the Department of Health, the forum offers a collaborative day of networking and knowledge sharing. Together, we will showcase survivorship care across Victoria, highlight innovative approaches and share practical strategies that support improved outcomes for cancer survivors.

The forum will include:

  • Welcome address
  • Department of Health update
  • Consumer advocate story
  • Presentations from the VICS Statewide Survivorship Program grant recipients
  • Interactive group discussions focused on:
    • insights into practice and processes for discharging patients who are ~5 years post-treatment 
    • practice and processes to prepare patients for end of treatment and the survivorship phase of care
