Site Closure Report: Governance review

Once a project has been closed at Peter Mac – that is, there is no further contact with participants and no further access to source data/medical records is required – the Principal Investigator must submit a Site Closure Report for the Peter Mac site.

Site Closure Report governance submission requirements

  • If Peter Mac is not the reviewing Ethics Committee, the Sponsor or Principal Investigator (or delegate) should submit the Site Closure Report Form for the Peter Mac site. It is sufficient to submit a copy of the report for the Peter Mac site as submitted to the external reviewing Ethics Committee.
  • If Peter Mac is the reviewing Ethics Committee, both the ethics and governance reporting requirements are satisfied by the submission of the Final Report to the Peter Mac Ethics Committee Please refer to the information provided on the Final or site closure reports - Peter Mac ethics review

Procedure for submitting the site closure report

  • Site closure reports may be submitted via ERM (Ethics Review Manager) or via email to This email address is being protected from spambots. You need JavaScript enabled to view it.. Please include the Peter Mac project number and the term ‘Site Closure Report’ in the subject line of the email.
  • The Site Closure Report must be signed by the Sponsor, Coordinating Principal Investigator or Principal Investigator (or delegate).
